Consent to Participate

Before you begin the survey, please read the following.

This is a research study being completed under the direction of Karen Jurasek, Service Quality and Training Coordinator at Cowles library. The purpose of this survey is to identify how well Cowles Library is meeting library users’ needs and what we can improve upon. If you have questions, feel free to contact the survey coordinator at 271-2903, or send an email to libsurvey@drake.edu.

Confidentiality: The respondent’s privacy is protected in several ways. First, only very indirect information is captured, which would be difficult to trace back to an individual. Second, everything possible is done to separate personal information from survey responses before the responses are stored. Email addresses are collected for incentive purposes only and are not saved with responses. Survey data is stored on a server at Texas A&M University (TAMU), which stands behind a firewall.

Participation: Participation is voluntary. There will be no consequences to you if you decide not to participate and you may withdraw from the survey at any time.

Risks and Benefits: The risks of participating in this study are minimal and the results of this study will provide information about the quality of library service and future planning needs.

Informed Consent: Because this is a web-based survey, you consent to participate by clicking on the survey link and electing to fill out the survey questionnaire.

PLEASE NOTE: You will be asked to indicate your major/program of study (students) or department/discipline (faculty and staff). Disciplines are listed in alphabetical order in the demographics section near the end of the survey. If you cannot immediately locate your major/program of study or department/discipline, please scroll through the list and select the closest match.
